Catholic Charities’ Encompass Family Health Home designated to be children’s health home

   A joint initiative of four upstate diocesan Catholic Charities agencies that will coordinate services for high-needs children launched Dec. 8 with an initial rollout in 37 counties, which will soon reach 48 of the state’s 62 counties. When the Encompass Family Health Home reaches full capacity, it is anticipated that it will serve 30,000 children per year.

   Catholic Charities of Broome County is the lead agency in the joint initiative, called Encompass Family Health Home, which was selected back in June 2015 by the New York State Department of Health as one of such 16 entities throughout the state. Partners include the Catholic Charities agencies of the Dioceses of Albany, Buffalo, Syracuse, and Rochester.

   Children served will include those with a serious emotional disturbance, medically fragile children, and youth served in foster care and through the juvenile justice system. Encompass will utilize a family- and youth-driven model of care to improve health outcomes for the most vulnerable in our state. Encompass is a collaboration of Catholic Charities and its partners across upstate New York and is bolstered by the support and blessing of the bishops of each diocese.

   “This designation says a lot about the quality of our Catholic Charities service network in New York State,” said Michael Lawler, Director for Catholic Charities for the New York State Catholic Conference. “It also highlights the incredible collaborative effort by the diocesan Catholic Charities agencies that went into bringing this initiative to fruition.”

   “Over the past year, the Council of Catholic Charities Directors has worked diligently to make coordination and collaboration amongst our eight dioceses a top priority as the State looks to transform its Medicaid system,” said Joe Slavik, President of Encompass and C.E.O. of Catholic Charities for the Syracuse Diocese.

   Annually, Catholic Charities provides $1.3 billion in program services to more than 1 million New Yorkers through its network of social services agencies in all 62 counties of the state.
